I assume you're a CLEC -- MA-17 is for services to CLECs, UNEs. ISPs and others have to deal with retail tariffs.

What will I need to order to do this? and what can I expect to pay for
this?  Are there any cheaper options?
Note that VZ recently filed a new set of UNE rates, which are not yet official, but which are supposed to be retroactive until last summer. In most cases the rates are substantially higher than Tariff 17 rates now in effect. This is before the DTE, which, while theoretically there to protect the public, typically functions as VZ's outside law firm. Litigation is possible if not likely; the DTE tries to temper VZ's wording in order to protect them from losing in federal court, while maximizing VZ revenue.

If the two DS3 terminals are not VZ wire centers, then you need DS3 loops and DS3 interoffice facility mileage (IOF). The IOF has a fixed and mileage component. However, you can't simply join them together -- in most cases, the only way to join DS3 IOF to a loop is via collocation, which is quite costly. There's an exception for EELs, but those are subject to usage restrictions, and I'm not sure offhand if DS3 can be an EEL. You can join the IOF to a loop at one end, if it's a CLEC location, by designating that loop as an Entrance Facility. The far end is the tough one.

Given that both the state rates and the FCC's UNE rules are in flux (final rules pending and not announced), it's not save to be too specific.
